7th Grade Immunization Requirements:
Students will not be able to attend classes if they have not received the required immunization.

Parents of 6th Graders 2022 – 2023 School Year
The Florida Department of Health requires 7th graders to have the following immunization:
Tdap (1 dose) Tetanus – Diphtheria – Pertussis
You must show documentation of the Tdap before your child starts 7th grade. (2023 – 2024 School Year.)
Current 6th graders should already have received the immunizations listed below:
- MMR (2 doses) • Polio (4 doses)
- DTP/DTaP (5 doses)
- Hepatitis B series
- Varicella vaccine (2 doses) or signed proof of the disease
Immunizations are available from your private medical doctor or the local health department (see list on the back of this page).
Take your child’s complete immunization record with you to obtain the vaccine. Contact the school office ahead of time to pick up your child’s immunization certificate.
Return the DH 680, Florida Certificate of Immunization, to the school after each immunization visit.
Your child must receive all the required immunizations (shots) to attend school in Florida. If your child does not have all the required shots, you must arrange for your child to receive the remaining ones.
You should contact your child’s health care provider to receive the immunizations. You may also call the Florida Department of Health in Polk County clinic nearest to you to have your child immunized. You will need to call for days and times when immunizations are available at the following locations.

Please note:
- Immunizations required for school attendance are available at the above listed clinics free of charge for children under the age of 19 that qualify.
- When going to a clinic for vaccinations, take your child with you and a copy of his or her previous immunizations.
- A permanent immunization certificate cannot be issued to you without a completed shot record.
